Total Gastrectomy for Early Gastric Cancer


Abstract
A clinical study was performed for total gastrectomy for stomach cancer at the Department of surgery, Pusan National University Hospital, from Jan, 1982 to Dec. 1991.
@ES The results obtained were summerized as follows;
@EN 1) Total gastrectomy was performed on 154 cases(16.6%) among the 1,032 cases of stomach cancer for 10 years.
2) The male to female ratio was 2.20 : 1 and the maximal incidence of age group was 6th decade.
3) The most common clinical findings were epigastric pain and discomfort.
4) The most common site of the tumor was body of the stomach, 113 cases(73.5%). The predominent histologic type of the adenocarcinoma was poorly differentiated type, 63 cases(40.9%).
5) Total gasterctomy with splenectomy was performed in 59 cases(38%), and radical total gastrectomy was performed in 54 cases(35.9%). Extended radical total gastrectomy was performed in 41 cases(27.0%).
6) As reconstructive procedure, loop esophagojejunostomy with Braun anastomosis was performed in 85 cases(55%), and Roux-en-Y esophagojejunostomy was performed in 69 cases(44%).
7) Major postoperative complication was occurred in 36 cases(23.3%) and the most common complication was pulmonary complication in 13 cases(8.4%). The most common late complication was reflux esophagitis in 45 cases(29.2%).
8) The operative mortality was 5.2%(8/154) and the most common cause of death was sepsis in 3 patients(1.9%) and pulmonary failure in 3 patients(1.9%).
9) The 5 year survival rate of patient with gastric cancer was 16% at TNM stage III, 33.3% at stage II and 80% at stage I respectively.
